The program aims to detect chronic conditions early through community-based screenings and follow-up care coordination. Eligibility is limited to healthcare providers with established rural presence and capacity to coordinate follow-up care. Applicants must demonstrate existing partnerships with local health organizations and provide a detailed plan for screening logistics and follow-up care. Oklahoma's rural counties will be prioritized based on the Oklahoma State Department of Health's Rural Health Index.
